(Information needs to be entered on The Arbiter within 24 hours after match by Referee of match)

1. Field conditions
2. Field manager present
3. Certified Trainer present
4. Information on any injuries
5. Goal scorers (player number and team name)
6. Information on yellow and red cards
7. Any trouble (coaches, fans, players, etc)
8. Weather related issues
9. Was the match on schedule
10. Anything unusual that should be noted during match play
11. Whether you received game payment at site or not
12. Any safety issues for referees or players

ALL RED cards must be called into Greg Pilbeam that day– 616-463-4630). Please call before 10pm (if after, please contact Greg immediately the following morning)
